How did this patient hear about Platinum Ridge? Prior patient

On 1/6/17, Ruth was admitted to Platinum Ridge Rehab with a diagnosis of general weakness resulting from a fall she had experienced at a personal care home she was living. Upon admission, she was requiring contact-guard assist for all transfers, dressing and activities of daily living. It was great to see Ruth working so hard daily with therapy to reach her goal of going home . She worked very hard alongside occupational and physical therapy to reach her physical and mental goals of health. Ruth was always very pleasant and upbeat and had the support of her daughter to which she was going to reside with after discharge. While at Platinum Ridge, she largely participated in activities that were offered through the recreational program. The team is thrilled to share that Ruth has been happily discharged home to be with her daughter and dogs!
