George was a cheerful, uplifting, gentleman that came to Platinum Ridge Rehab for therapy services after a stay at the hospital with ascites (swelling of abdomen due to liver disease). George also had severe swelling in both legs of which impeded his ability to walk and get out of bed on his own.
Upon evaluation, George needed Moderate assist (50% assist) to get out of bed due to increased swelling and pain in his legs. He required Stand-by assist (not physically touching, standing close by for safety) as well as for pivot transfers and walking up to 15 feet with a wheeled walker. He was unable to complete steps at the time. George was distant supervision for eating, hygiene, toileting, upper body dressing. He was Min. Assist (requires 25% from
therapist) for lower body dressing and meal prep due to swelling and fatigue with mobility at this time. George was a high risk for falls.
George was very motivated to get better and return home with his daughter with hopes to get his own apartment in the near future. He worked very hard in both Physical and Occupational therapy with weight training, transfer and gait training, as well as activities of daily living to be able to return home and care for himself. He worked on endurance and balance training to increase his ability to walk further distances with decreased assist as well as
simulated IADLs in the kitchen for increased independence with tasks.
George did wonderful while at Platinum Ridge for therapy. At discharge he will be able to walk up to 300 feet with a wheeled walker as well as be able to ascend/descend 12 steps with a handrail. He was also able to do all hygiene, toilet transfers, upper and lower body dressing, and meal prep at time of discharge.
George is the prime example of what motivation and determination can do to help you achieve your goals. He surpassed his expectations while completing his inpatient stay at Platinum Ridge. He was set up with home health services to continue on his path to healing and to work towards his goal of getting his own place. We wish you the best of luck George!
